Transition From Early Intervention
If your child is currently receiving Early Intervention services and a potential disability is suspected, a collaborative transition plan is established between the Early Intervention agency and District 123. This ensures the provision of appropriate services without interruption when your child turns three, supporting their ongoing developmental needs. For more information, please review the Family Transition Workbook. (Spanish)
If you have concerns about your child's development between birth and three years old, early intervention services can offer valuable support. Please contact the Illinois Department of Human Services to explore these resources to help your child thrive.
