Survey Open until Apr. 2, 2021
Teachers, parents, and students across Illinois have an opportunity to participate in the annual statewide Illinois 5Essentials Survey. This survey is designed to generate a detailed picture of the inner workings of District 123 schools.
As a parent, this opportunity allows you to share your thoughts on the important elements of school effectiveness in a survey about your child’s D123 school.
On behalf of the Illinois State Board of Education, the Illinois 5Essentials Survey will be administered online by UChicago Impact at the University of Chicago. The survey gathers data related to five indicators that can predict important student outcomes. These five indicators that affect and predict school success are:
- Effective Leaders
- Collaborative Teachers
- Involved Families
- Supportive Environments
- Ambitious Instruction
Survey responses will never be connected to the identity of you or your child. School reports will only present aggregated information of how parents and students responded as a whole.
Student Survey Information
In addition to the parent surveys, fourth through eighth grade students will be asked to participate in a 5Essentials student survey sometime between Feb 3 and Apr 2 If you DO NOT wish to have your fourth through eighth grade student(s) participate, please complete this Form and return it to your child’s school office.
The Illinois 5Essentials Survey: Organizing Schools for Improvement was developed by the Illinois State Board of Education (ISBE). The survey’s staff and student results will become part of the 2020 Illinois State Report Card for your child’s school. Please be aware that under the Protection of Pupil Rights Act. 20 U.S.C. Section 1232(c) (1) (A), you have the right to review a copy of the questions asked of your student(s).
